Advancing Implementation & Care Coordination in Health & Social Service Systems

This presentation will showcase challenges and opportunities to improving the implementation of best practices and the coordination of high-quality care in both health and social service organizations. Presenters will share their work developing provider- and organizational-level strategies to (a) support best practice implementation across the stroke rehabilitation care continuum and (b) facilitate access to substance use treatment services in child welfare agencies.
